How We Deliver Care
- 13
Dialysis at PB Health is not a standalone service. Our unit functions as part of an integrated care model that links dialysis nurses, visiting nephrologists, general physicians, dietitians, and care coordinators under one roof.
- 14
Every patient follows a structured care pathway: from first assessment and access planning through regular dialysis sessions, dietary management, and complication monitoring. When specialist review or tertiary input is needed, we coordinate promptly so that continuity is never broken.
- 15
We are especially focused on early detection and proactive management, meaning patients with advancing chronic kidney disease are supported before dialysis becomes an emergency.
